This form is intended for the assessment of admin staff. You are to take into account the performance of the relevant admin staff over the year, and any improvements from the last assessment. The criteria contained in the form are not intended to be exhaustive. Views should be expressed on any aspect of the admin staff’s performance and development that are considered important. The form asks for the admin staff to be rated across a number of different dimensions. Please use the Rating Scale below to guide your assessment.

1. Consider all the work/ output, or performance that you receive from the admin staff. Consider its overall quality – is it devoid of mistakes? Is it of a high standard? Please rate the overall quality of the work/ output, or performance.

2. Timeliness – does the admin staff complete his or her work promptly and need not be ‘chased’ for output?

3. Consider the overall work attitude of the admin staff – is he or she proactive and dedicated? Is he or she pleasant to work with? Is he or she cooperative with colleagues? Overall, does he or she have a positive attitude towards work and the firm? Please rate the overall attitude of the admin staff.

4. Punctuality, attendance and dependability - does the admin staff arrive on time? Is the admin staff consistently available and on task as required by job description, assigned tasks, and colleagues, and will not go ‘MIA’?

5. Initiative - does the admin staff proactively seek solution to problems? Can the admin staff work with minimal direction, anticipate and meet organizational needs?

6. Personal management - Does the admin staff have effective organization and planning skills e.g. is the admin staff’s papers and files, where relevant, kept in an orderly fashioned? Does the admin staff keep proper records?

7. Consider the overall performance of the admin staff for this period, along all the dimensions above and taking into account any other relevant factors. How would you rate the admin staff – is he or she a superior performer, a strong/ good performer, an average performer or a poor performer? (Refer back to the rating scale set out in the introductory section of this form.)
